Request Your Cumulative Credit Report

To Request a Cumulative Report of CME Credits, Please follow the applicable steps below.

  • Please allow a 48-hour turnaround time to receive your report.
  • If you do not receive your report within 48 hours, please call 843-876-1925.

MUSC Employees

MUSC Faculty/Staff will receive one complimentary cumulative credit letter per fiscal year. Any additional credit letters will be offered at a fee of $25 per letter.

Please complete the form below using the name of the person whose name will appear on the credit letter.

Non-MUSC Employees

You may obtain your report for a $25 fee. Please call our office at 843-876-1925 with credit card information available.

Please Note: If you are requesting a report including credits for multiple years, all the requested years will be listed on one cumulative credit letter. If a separate credit letter is needed for each year, a fee for the extra letter(s) will be charged $25 per letter.
