• While practicing in SC, you need a current medical/dental license and a current SC DPH to be eligible to apply for the federal DEA license.

Application/Renewal Instructions

Two Types: Limited or Permanent

A Limited medical license is restricted to those contracted in a residency or fellowship and requires supervision. It is renewed annually. This is the minimum requirement for residency/fellowship training. Please review the SCLLR website for full requirements to obtain a limited license.

A Permanent medical license is an unrestricted license for fully qualified physicians and is required for all independent moonlighting activities. It renews every two years. Please review the SCLLR website for full requirements to obtain a permanent license. 

   

How to Apply
Limited License

  • Go to this link.
  • Select Licensure as a Limited MD or DO Application (6 mos or 1 year) Requirements and Forms. Review the requirements and print out/complete any applicable forms. The 1-year licensure option is recommended.
  • Return to previous screen, click on 2. to begin the completion of the online application. Note: The GME Office will submit to the SC Medical Board, the three Letter of Recommendation (LOR) writers, the contact information needed for your license, and verification you will be on an MUSC contract. The GME Office will also notify all Program Coordinators to submit the Supervising Physician Form on your behalf.
  • For the LOR writers, please list:

Tips for completing the application

  • For the mailing address, please include the Mail Stop Code (the GME Office is MSC 333) or use your department Mail Stop Code.
  • Do not list your home address as your practice address, as this information is public knowledge and is listed on the SC Medical Board website if one looks up your name.
  • You are responsible for making sure the following forms are printed out, sent to the applicable agency(ies) and submitted back to the SC Medical Board:

    - Verification of Licensure form. Only applicable if you were ever licensed in another state
    - Certificate of Medical or Osteopathic Education form. Print out and submit to your medical school for their completion and submission to the SC Medical Board

  • You may begin the medical license application without a social security number. You may use the number 999-99-9999. Once your social security number is issued, go back into your application and update that section.

Permanent License

  • Go to this link.
  • Select Licensure as a Permanent MD or DO Requirements and Forms.  Review the requirements and print out/complete any applicable forms.
  • Return to previous screen, click on 2. to begin the completion of the online application.
  • Other than the forms you must send out for completion, the whole process of obtaining a Permanent SC medical license is online only. 

How to Renew
Limited License

  • Go to this link.
  • Click on the red banner indicating Document Submission. Do not click on Renew My License.
  • Log in and select Add Documents to this credential.
  • Select one year renewal.
  • Complete renewal form after printing out the form and upload the document into your portal.
  • Note: GME will submit confirmation of your contract.

Permanent Medical License

  • Go to this link.
  • Click on Renew My License.
  • Log in to begin the renewal process.

  • Other than the forms you must send out for completion, the whole process of obtaining a SC medical license is online only.

How to Renew

GME Office pays for initial and subsequent renewals
Your medical/dental license must be issued before your SC DPH can be issued.

Medical: Valid for October 1 to September 30
Dental: Valid for April 1 to March 31

How to Apply
New applications are processed through the GME MedHub onboarding package. Please contact the GME Office for specific questions.

How to Renew
Renewal forms will be distributed by the GME office annually around August (February for dental residents).

While in training, MUSC residents and fellows qualify for fee-exempt DEA registrations, at no charge. The fee-exempt registrations are only valid for use at MUSC or within the training program.

The GME Office will provide instructions to eligible applicants.

 

DEAs and moonlighting outside of MUSC
