Event Intake Forms

We welcome internal and external interest in our event planning services. Whether you are affiliated with the Department of Psychiatry and Behavioral Sciences or are an entity external to the Medical University of South Carolina, we are here to help.

For internal stakeholders, please be advised that these intake forms and subsequent communications serve three purposes:

  • To create a streamlined communication process with the MUSC Office of Continuing Medical Education OCME and/or the MUSC Office of Development
  • To implement best practices to ensure your event will be successful and represent the Department of Psychiatry and Behavioral Sciences appropriately
  • To provide a greater awareness of the important work you are doing

We are excited you are interested in planning a continuing education event. Our office is the Department-level liaison that you will need to work with to secure Department-level and Enterprise-level accreditation for your event to represent the Medical University of South Carolina and the Department of Psychiatry and Behavioral Sciences.

CECAR Grievance Form

The Medical University of South Carolina (MUSC) is fully committed to conducting all activities in strict conformance with the American Psychological Association's Ethical Principles of Psychologists. MUSC will comply with all legal and ethical responsibilities to be non-discriminatory in promotional activities, program content and in the treatment of program participants. The monitoring and assessment of compliance with these standards will be the responsibility of the Program Administrator.

While MUSC goes to great lengths to assure fair treatment for all participants and attempts to anticipate problems, there will be occasional issues which come to the attention of the program staff which require intervention and/or action on the part of the program staff or an officer of MUSC. This procedural description serves as a guideline for handling such grievances. When a participant, either orally or in written format, files a grievance and expects action on the complaint, the following actions will be taken.

To file a formal grievance, complete the CECAR Grievance Form. Please type or print legibly and you must sign the form. Submit grievance to milanak@musc.edu.

Our office may use the information you provide to contact you as we expect you to remain engaged in the review process. You may withdraw your complaint in writing at any time.
