To request a duplicate certificate of participation from a past conference, seminar, or Grand Rounds Series presented by the Department of Psychiatry & Behavioral Sciences, please complete the below form. You may obtain a duplicate certificate for a $25 fee per request.

If you need certificates from more than one conference or Grand Rounds Series, please return to this form and submit a separate request. Please allow a 48-hour turnaround time to receive your report.
